Player Paperwork

To be allowed to participate, each player (football and cheering) must return the following documents and have them submitted to their head coach.

To print the required documents, select a document type to open (PDF, MSWORD, HTML) and click each of the links in that column to open the document for printing.  PDF or MS Word format provide the best printing results.  The Adobe PDF Reader can be downloaded here and can be used to print the PDF version of these documents.

PHYSICALS:  A copy of a signed doctor's physical is acceptable in place of having the information filled in on the Physical Form.  The Contact Information at the top of the document must be filled out though by a parent/guardian.  Also, please include any special medical information that coaches should be aware of (inhalers, epi-pens, insulin, recent injuries, etc).  Physicals must not be more than 13 months since the date of physical at any time while participating on the team.  The 13 month requirement allows parents whose insurance only covers one physical per 12 months to schedule a physical that overlaps with our requirements.  Flag Football and Flag Cheering only require a physical to be turned in the first year that the child participates.  Pee Wee and Jr High Football and Cheering require a yearly physical.

BIRTH CERTIFICATES: The copy of the notarized birth certificate for Junior High Football is due before the athlete may participate in any game.
